Vision Care Benefit

The Merit Benefit Plan's Vision Care benefit is self-insured.  Claims are adjudicated by Great-West Life.

The Merit Benefit Plan's Vision Care benefit provides the following coverage, up to the stated maximums, while the participant remains eligible for benefits.

Eyewear/Laser Eye Surgery

  • For employees and dependents: lenses (including anti-reflective or anti-scratch coating), frames, contact lenses, sunglasses, prescription safety glasses or laser eye surgery, when prescribed by a physician, ophthalmologist or optometrist to a maximum of $350 every two calendar years for participants age 19 and over, and $350 every calendar year for participants who have not attained their 19th birthday;
  • For employees only: prescription safety glasses when prescribed by a physician, ophthalmologist or optometrist, to a maximum of $150 every two calendar years (in addition to the $350 benefit every two calendar years outlined above);
  • For employees and dependents: visual training when prescribed by a physician, ophthalmologist or optometrist, to a lifetime maximum of $200 per person;
  • For employees and dependents: contact lenses prescribed by a physician, ophthalmologist or optometrist, if considered medically necessary (e.g. for severe corneal astigmatism, severe corneal scarring, keratoconus or aphakia) and required to improve vision in the better eye to at least 20/40 if this is not possible without conventional glasses, to a lifetime maximum of $500 per person.

Eye Exams

  • For employees and dependents: eye exams by a licensed ophthalmologist or optometrist where not covered by a provincial medical plan, to a maximum of $75 every two calendar years for participants age 19 and over, and $75 every calendar year for participants who have not attained their 19th birthday.  No payment will be made for eye examinations that are medically required.
